Maternity Notes Review

We all know that medical notes can be difficult to navigate, never more so than now, as care providers move towards electronic documentation. Sometimes we don’t fully comprehend the events of our pregnancy and birth care; we may have unanswered questions or simply need a comprehensive timeline. Whatever the case, an independent review can help to address those issues and help you to understand why events transpired as they did.

To undertake a review I need full access to a copy of your notes and it can be helpful if you outline any specific concerns or questions that you have to enable me to fully address them. I compile a full timeline of your care (allowing you to access events easily) alongside a report to highlight where your care was appropriate, what might have been done differently and what the implications might be should you decide to conceive again.

A notes review is merely that, a review of the documentation as recorded by staff at the time of the event. It can’t reflect your personal feelings or experiences which are just as important, if not moreso, than the clinical events. Because of this, once the report is completed it’s advisable that we meet, allowing you to read it in a safe space and to ask any further questions that might arise. Sometimes the report can be triggering or upsetting and I feel it’s important to feel supported and listened to if that is the case.
